99% Invisible did an episode on billing
[edit]99% Invisible did an episode on title sequences in movies and, especially, the billing. It would be a good thing to incorporate to this page.
It refers to "stacked" billing as "Laverne & Shirley" billing. It also mentions a controversy with billing on "Outrageous Fortune", where to satisfy both stars, two different prints of the film were made, one with Shirley Long's name first and the other with Bette Midler's first!
