On May 31, 1985, A Grumman G-159 Gulfstream I operated by General Aviation inc. from Nashville International Airport to Indianapolis International Airport, Crashed shortly after takeoff due to a engine 1 shutdown causing a stall and crash near Nashville International Airport and killing all 2 crew on board.[1]

The aircraft involved was a 27 Year-old Grumman G-159 Gulfstream I and operated by General Aviation inc. while it is being operated by Orion air with Rolls-Royce Dart Mk.529 engines.[2]

Accident

While the plane took off at nighttime the plane lifted off at 21 seconds while in the ground and after that suddenly a Engine failure was cause by Pilot error(Human error) and 33 seconds after take off the plane which was a Grumman Gulfstream I crashed shortly after takeoff killing all 2 occupants because it was a Cargo variant and not a passenger flight.[3]

The accident involved with 2 crew on board and a Cargo flight to Nashville to Indianapolis and all 2 crew where all American Nationalities, The pilot in-command was American Captain Gary F. Lewis, And The First officer was American Charles R. Sinks.

Investigation

The Federal Aviation Administration said the plane had an Engine failure by Pilot error and crashed near Nashville International Airport after 33 seconds after takeoff, They reported that they found there was 0 survivors and 2 crew killed, They also reported that they were heading to Indianapolis International Airport or Indianapolis which was the capital city of Indiana.[4] And the Flight recorder was recovered including the Flight data recorder.[5]
